Liver Changes in Pregnancy Made Easy

One of the most appreciated sessions this month was “Liver Changes in Pregnancy Made Easy” by Prof. Dr. J.B. Sharma, a topic that frequently appears in DNB, MS, and residency examinations but often creates confusion due to the overlap between physiological and pathological changes. 

The session simplified a complex subject by helping residents understand what constitutes a normal liver-related change in pregnancy and how to differentiate it from genuine liver pathology. 

Key concepts covered included: 

  • Physiological liver and hematological changes during pregnancy 
  • Why pregnancy is a hypercoagulable state and its clinical significance 
  • Understanding changes in plasma volume, albumin levels, and hematocrit 
  • Classification of liver diseases during pregnancy 
  • Liver diseases unique to pregnancy versus coincidental liver disorders 
  • Conditions such as intrahepatic cholestasis of pregnancy, HELLP syndrome, acute fatty liver of pregnancy, and pre-eclampsia-related liver dysfunction 
  • Approach to abnormal liver function tests in pregnancy 
  • Interpretation of ALT, AST, ALP, and bilirubin during pregnancy 
  • Role of ultrasound, MRI, and imaging in evaluating liver disease during pregnancy 

A particularly useful aspect of the discussion was the practical approach to interpreting abnormal liver function tests in pregnant patients—an area that residents frequently encounter during both examinations and clinical postings. 

Rather than presenting isolated facts, the session connected physiology, pathology, investigations, and clinical decision-making into a structured framework that residents can apply directly in practice.

Reviewed conservative management options and life-saving interventions used in postpartum hemorrhage.

A practical discussion on safe abdominal entry techniques, complications, and preventive strategies during laparoscopic surgery.
